If you pull the lever, you kill 1 but then the trolley turn around and kill the five. The five of them lived in horror and they realised they're not saved. You can choose not to pull it but the result stayed the same but the trolley is slowed significantly from running over the 5 so now the 1 person gotta die slower and more painful.
