Fanfic, unless given the go ahead by the creator.
example: Star Wars extended universe novels approved of by George Lucas himself.

No. They're similar but not the same.

Fanfic is like its name implies. It's a fiction written by a fan of a particular story, using the original characters, places, or other elements and using them in a different way.

Spin-off is similar in nature. But this one is usually written by either the author themselves or someone authorized by the author.

Personally I feel anything set in an existing universe or using existing characters created by someone else is fanfic.

As for wider perception of this it often depends on the original creators perspective and intellectual property rights. For many if the original creator endorses the other creator's work on the universe or if the original creator sold the intellectual property rights to the new creator then it is no longer fanfic.

It can get even blurrier when you have things that started as fanfic, but were then reskinned to be "original" such as 50 shades. It started out as fanfic, but now many consider it original content because the author changed the names and some other things to make it legally distinct from twilight.

If it was written by another author without the input of the original, then fanfiction. If it was approved by the original, then probably not.
