Oh, this might be an old bug. I had to republish my novel for that cover to work. Not entirely sure what caused it to happen, but I know there was a bug report somewhere on this forum that covered it; lots of folks were talking about how to get around it because changing covers also caused some...

Never encountered that issue before. Is it possible that the account you're trying for may have initially been a banned one by a different user? That's a big stretch of probability, but not sure why else it would be failing.

Yep, pretty much.

You'd have to email an appeal. I would not be surprised if someone, using multiple accounts, filed falsified reports maliciously. Not saying that is what happened here, but it is a possibility that can happen.

Best guess is the boom of AI giving those folks who want to direct more than write stories. Also the gradual decline of the number of active moderators. Plus Tony can only do so much alone. It was very active in the beginning and up to 2020, we even had contests with prizes, but after that...

It had more usage when the traffic of engaging readers was higher than the authors here.

Not too sure, but it might be "Night of the Soulless Heathens"? He ain't after pages, but coins. Everything else sounds pretty close to what you described.
